Is it possible to create one template that can be used by the facilities management department to measure end user satisfaction with multiple processes/projects?

I am trying to create a template for measuring end user satisfaction at sites. I would want a template that would require the user’s rating and sign off. Is it possible to create one template that can be used to measure satisfaction with multiple processes/projects?
